2. OF 612 - NAF Application Continuation Sheet (if applicable)

3. Veterans' Preference Worksheet
   (required even if not claiming Veterans' Preference)

4. SF 15 - Application for 10-Point Veteran Preference
   (required only if claiming Veterans' Preference)

5. Letter of Disability from the Veterans Administration dated within the last year (if applicable)

6. Military Spouse Preference Worksheet
   (required even if not claiming Spouse Preference)

7. AF 243 - Statement of Physical Ability

8. OF 306 - Declaration for Federal Employment

9. Proof of Age *
   (Birth Certificate, Driver's License or other governmental form)
   *Required only if applying for a job that serves alcoholic beverages, or for childcare positions, including CDC or Youth activities.

To apply for a Childcare position, including CDC or Youth Activities, these additional items are required:

1. Proof of Age
   (Birth Certificate, Driver's License or other governmental form)

2. Copy of High School Diploma or Equivalent GED

3. Copy of a Current Driving Record *
   North Dakota records can be obtained online: https://secure.apps.state.nd.us/dot/dlts/dlos/welcome.htm
   * Check of your driving record is required as a condition of employment. You are responsible for obtaining a copy of your driving record at your own expense. The Driving record needs to be current within 90 days of the date of the application. The driving record must be renewed every 90 days to be allowed to stay on file.
